Alternate Address Form

Complete this form if you have two different homes and are at them the same time each year.

Application for Dues Relief

This form is used by a Daughter 85 years or older, who is in deteriorating health, to apply for relief of her obligation to pay dues. It must be signed by her clergy and chapter president, or Diocesan or Provincial president if she is a Daughter at Large.

Application for Junior Membership

Junior Directresses may use this form for prospective members.

Application for Reinstatement

Use this form when a Daughter needs to be reinstated. Please submit the application with the annual dues and nominal fee.

Application for Senior Membership

This form is completed by prospective members, signed by the clergy and chapter president and then given to the Study Guide Leader for submission.

Chapter Officers Form
Complete this form when officers change in your Chapter.

Diocesan Assembly Officers Form

Complete this form when officers change in your Diocesan Assembly.

Disposition of Cross Card
The Disposition of Cross Card is used by a Daughter to convey her wishes as to what should be done with The Order's cross upon her passing.

Junior Directress Update Form

To be used when Directresses contact information changes or when Directresses change or when a Junior Chapter becomes inactive.

Provincial Officers Form

Complete this form when officers change in your Province.

Transition Form
Use this form to report a change of name, address, transfer to new chapter or to a Daughter at Large, resignation or member’s death.

Pray Your Way into Their Story

This “Pray Your Way into Their Story” booklet will help prospective members, as well as all Daughters, explore how Service and Evangelism is an essential result of Prayer.  The phrase, “Prayer-Care-Share” summarizes how our commitment to pray, serve, and evangelize can flow into each other as a continuous circle. This booklet is used with National Study Guide or as additional chapter study resource.

The Spiritual Formation Guide

Continuing the Journey: The Spiritual Formation Guide is a celebration of life in God through prayer, study, and discernment. you are encouraged to use this material as presented or as a supplement to chapter meetings or quiet days.

Junior Directress Manual

The Directress Manual contains everything a senior Daughter or Directress needs to know to start or maintain a Junior chapter.  It also contains a lot of helpful hints, suggestions and ideas. The Manual is informative, inspirational, and empowering.

Junior Directresses may use this form for prospective members. Please submit the completed form with a completed Order Form.

Leader's Guide for the Primary Grade and Youth Junior Daughters Study Guide

This leader's guide is a combination of both the Primary Grade and Youth Study Guides. It can be used to lead prospective Junior members through either or both study guides.

Primary Grade Junior Daughter Study Guide

This study guide is used for prospective Junior members for ages 11 and under.

Youth Junior Daughter Study Guide

This study guide is used for prospective Junior members for ages 12 and over.

National Transition Study Guide

This National Transition Study Guide can be used by Junior Daughters transitioning to Senior Daughters.

Junior Directress Update Form

To be used when Directresses contact information changes or when Directresses change or when a Junior Chapter becomes inactive.

Transitioning Junior Update Form
To be used to collect information about a newly transitioned Junior Daughter. Please send form to JrDaughtersChr@doknational.org.

Jr. Daughter Chapter Activity Form
To be used to collect information on Jr. Chapter activities and service projects in order to be shared with other Directresses. Please send form to JrDaughtersChr@doknational.org.

Acknowledge, Waiver and Release from Liability and Hold Harmless Form

This form is used when a Junior Daughter is going on a trip. It is to be completed by a parent and can serve as a permission form. It is a release from liability form.

The Medical Information Form

To be completed by a parent of a Junior and is used to collect information about any medical conditions and/or allergies and what to do in the event of an emergency.

The Medical Release Form

To be completed by a parent and is used to provide permission to the Directress to provide medical care to the Junior in the event of an emergency.

National Council Nominee Form

Each Diocesan Assembly may nominate one member of the Order for election to National Council, with the member's consent. The Deadline for nominations is January 15, 2015.

Triennial Registration and Credentialing Form

Registration for Triennial 2015 opens on January 1, 2015 with a deadline of May 31, 2015. Each chapter, diocese, and province is allowed one delegate and one alternate.  Delegates must submit a signed form to the National Office.  Please note further instructions on the form.

Triennial Exhibitor Form

Deadline for Exhibitor registration is May 1, 2015.
